New Patients:  For appointments, we require a referral from your primary care provider.

Existing Patients:  To make an appointment, contact our office at (803) 256-3534 Monday to Thursday from 8:30 AM  – 12:30 PM or 1:30 – 4:00 PM and Friday from 8:30 AM – 12:30 PM.

All Patients:  Please bring your picture ID, insurance card and copay, medication list and all lab results and images obtained at outside offices to each visit.  Our office will also ask you to verify your personal information at each visit.

No Show Policy:  Patients that do not show up for a scheduled appointment, or who have not contacted the office within 48 hours of their appointment time to cancel, will be considered a “No Show.”  After you have “No Showed” for two appointments within a 12 month period, this may result in dismissal from this practice.
