Laurel Endocrine and Thyroid Specialists require that certain forms be completed at the initial consultation and again at each office visit.  These forms are available to print or submit online.  The online forms allow you to fill the form out online and submit directly to our practice.  You will also get an email with the completed form attached for your records.  Please either print out these forms, fill them out and bring with you to your appointment or complete them online and submit them to our office.
