Office Policies
At Laurel Endocrine and Thyroid Specialists, it is our mission to provide our patients with quality care while maintaining a professional attitude. As endocrinology specialists, we understand the growing need for endocrine care and we endeavor to meet the needs of this expanding field. The following office policies help us to maintain and achieve our goals.
Appointments: Our practice requires a referral from your primary care provider if you are a new patient or a patient who has lost contact with our office for an extended period of time. For established patients, please contact our office to schedule an appointment.
No Show Policy: Patients that do not show up for a scheduled appointment, or who have not contacted the office within 48 hours of their appointment time to cancel, will be considered a “No Show.” After you have “No Showed” for two appointments within a 12 month period, it may result in dismissal from this practice.
Prescriptions: Our practice discusses prescription refill requests during office visits. If you need a refill on a prescription between appointments, we are usually able to process requests within two business days. If you, your representative, or a pharmacy request a prescription outside of your office visit, there may be a $10.00 charge.
Medical Records: We strongly adhere to HIPPA regulations to maintain the confidentiality of our patients’ medical records. Our practice requires patient consent before records can be released to the patient, insurance companies, law offices, or any other entity. Please contact the medical records personnel for further information.
Billing and Insurance:
Co-Pays/Balances: All co-pays, co-insurances and previous balances are due at check-in. If you are unable to make your payment in full at the time of service, then a $15 service charge will be applied to your account. Failure to pay in full may result in your appointment being rescheduled.
Cancellations/Reschedules: If you do not cancel or reschedule a follow-up or education appointment 24 hours before the appointment time, then a 50.00 fee will be assessed. If you do not cancel or reschedule a consultation, initial visit, hospital follow up, or in-office procedure appointment 48 hours before the appointment time, then a $100.00 fee will be assessed.
Balances: All balances due from the patient are payable immediately. If you are unable to make payment in full, please speak to a member of our staff to make financial arrangements prior to your appointment.
Insurance: Our office files insurance as a courtesy to our patients. Please bring your card with you to each visit. If you have insurance but cannot produce a valid card, you will be considered a “self pay” patient and our office will expect payment in full at each visit until you produce a valid card. Our office will not file insurance on services over 45 days old.
